Comorbidities Seen with ANCA-Associated Vasculitis

The damaging potential of many inflammatory disorders is magnified by well-known associated comorbid conditions. This is particularly true in lupus, rheumatoid arthritis and psoriatic disease.

Englund and colleagues have analyzed a population-based cohort of patients with ANCA-associated vasculitis (AAV) followed for 12 years in Southern Sweden. Each patient with AAV (n = 186) was compared to four age- and sex-matched reference subjects to calculate the rate ratios for comorbidities. 

The most common comorbidities seen included osteoporosis (rate ratio 4.6, 95% CI 3.0–7.0), venous thromboembolism (4.0, 95% CI 1.9–8.3), thyroid diseases (2.1, 95% CI 1.3–3.3), and diabetes mellitus (2.0, 95% CI 1.3–2.9). No linkage was seen for ischemic heart disease or cerebrovascular accidents.

Not surprisingly, vascular and steroid related comorbidities were common. The association between AAV and thyroid disease merits further study.
