As the consultations succeed, it is common to for us to focus the reasoning on what seems obvious: patient with rheumatoid arthritis and pain = disease activity = therapeutic optimization. This leads to a high and undue use of immunobiologicals. I always advise my residents: when something does not evolve as we expected, take a step back to see the forest and not just the tree. We must be very careful with numbers and with the mathematization of medicine, after all, the scientific evidences of today will not necessarily be those of tomorrow. Nice comment by David Theis too.
