Psoriatic arthritis

      For people living with psoriatic arthritis (PsA), the comfort of a good night's sleep may feel out of reach for many.
      The last day was jammed with important reports and research including the late breaking abstracts.   
      A few things we know about opioids: use is common; it’s not efficacious in the treatment of spondyloarthritis; and, there is an opioid epidemic and its users are stigmatized. Abstract 0402 is a retrospective study that looked at patients with psoriatic arthritis or ankylosing spondylitis in the FORWARD databank. It sheds new light and leaves us with some food for thought
      There has been a lot of talk lately about the difference in the way male and female PsA patients experience their disease.  Are the differences only cultural or are there important clinical differences in the disease presentation, organ involvement and complications that physicians should be aware of?   
      Vaccine efficacy remains an important and highly discussed topic at this year’s annual meeting given the ongoing COVID-19 global health crisis, current influenza season, and recently published 2022 ACR guidelines for vaccinations in patients with rheumatic and musculoskeletal diseases.
      Day two at ACR 2022 was full of great sessions on imaging, vasculitis, lupus, vasculitis, spondyloarthritis, COVID, pregnancy, microbiome, economics and more. Here are the RheumNow faculty selections for #ACRbest abstracts today:
      An interesting study presented at ACR22 looked at whether neural networks can distinguish seropositive rheumatoid arthritis (RA+), seronegative RA (RA-), and psoriatic arthritis (PsA) using hand MRI data based on the structural inflammation patterns.
      Minimal Disease Activity criteria are used to evaluate PsA disease activity and response to treatment. Given that a patient only has to meet 5 of the 7 criteria, should the individual components be weighted equally? 
      It’s widely accepted that Psoriatic disease has multi-system, multi-domain potential that can lead to life altering complications if left untreated or undertreated. Rheumatologists have shifted the way we discuss treatment options and disease state expectations with patients. Two #ACR22 abstracts stood out to me regarding the Psoriatic disease patient experience and will, ultimately, add to what I do in clinic. 
      Recently the FDA approved deucravacitinib, a highly selective TYK2 inhibitor for psoriasis. Trials are positive in psoriatic arthritis and a phase II study in SLE. What about the effects? Presentations from the ACR22 meeting may provide answers.
      Despite a wealth of treatment options, a number of notable unmet needs remain in PsA, most notably the significant diagnostic delay that many patients endure. We are all aware that those with psoriatic skin disease are the most at risk cohort for PsA. Identifying patients with psoriasis who have, or are at increased risk of PsA, appears to be the low hanging fruit that we just can’t fully seem to grasp.
      The pharmaceutical companies have will showcase their featured clinical trials and abstracts at ACR 2022.  These are their best studies for you to review and evaluate as part of your to-do list. In the least, you should be familiar with the names and objectives of some of these studies as they will be discussed and mentioned throughout the meeting. Abbvie
