In my experience ENA profile may yield positive results in few cases when ANA is negative by IIF. One scanario is low titre anti-Ro antibodies: ENA profile may be more sensative in detection of these. Another scanario is when staining is cytoplasmic and laboratories may not report these e.g. In case of anti- Jo1 antibodies.
