Nice quick read on periop RA management.
Suspect we’re treating the surgeon more than the pt. by holding bioDMARDS, but observational data supports increased infxn risk and this algorithm seems reasonable

Namilumab (mono ab to GM-CSF, Granulocyte-macrophage colony-stimulating factor) with good efficacy among MTX/TNF inadequate responders in a phase II RCT.
NNT = 3-4 at week 12 for ACR50 at max dose. Some respiratory ADE but nothing major.

Phase 3 study tanezumab (anti-NGF) 2.5 mg or 5 mgin knee or hip OA find that 5 mg met 2/3 of 3 primary endpoints at 16wks; but the lower dose failed to be superior to NSAIDs in this 1 yr study. Safety outcomes where less favorable w/ tanezumab. https://t.co/l4Qffdn1nl

Comparing fibromyalgia, PsA, Psoriasis Pts - FM has more entheseal tenderness (92%) than PSA (66%) and PSO (59%). However with ultrasound, entheseal involvement were more frequent in PsA & psoriasis (~90%) vs FM (75%). What? US enthesitis in FM? https://t.co/wNBQEB3j0O

Lateral wedge insoles are often used medial knee OA. Study of 63 preselected pts shows a significant pain reduction (vs neutral soles) but mean difference was 0.7 on 0–10 scale; thus benefitting only a minority of pts https://t.co/7SK7Cl1Uft

TARA study takes 189 #RA pts in remission on combo Rx and tapers either the csDMARD or biologic. At 1 year, flare rates were similar 33% (DMARD) vs 43% (biologic). Tapering TNF inhibitors was superior, but would have saved more $$. https://t.co/XxZbx5zjCC
