Dr. Eric Yen presenting about lupus nephritis mortality in the USA (1999-2019):
- Increasing trend in LN mortality x5 yrs
- Increased odds of death for Black pts > Latinx > API > AI/AN
- Urbanization (large central metro area) independent RF + effect modifier

#ACR21 #Abstr0456 How can we explain cognitive impairment in #lupus? Using f- and DCE-MRI, a study showed:
💠Abnormalities in brain functional connectivity
💠Extensive Blood-Brain Barrier leakage (BBB)
BBB could be a future target @RheumNow https://t.co/KLeWfbQUQ0 https://t.co/ibNvBUWlMK

So what’s interesting is that type I IFNs actually tighten the BBB. Is this dysfunctional in SLE? Or does this argue that CI does not correlate with SLE disease activity perhaps? #ACR21 #ACRAmbassador https://t.co/y5x2k3NQFi

Why is lupus nephritis mortality trending up in the US, in contrast to overall SLE mortality?
Race matters
Urban location can make things worse
I'm outside looking in but hope it's not too bold to say disparities affect whether you live or die from LN

SLE patients with +aPL was associated with:
➡️Nephritis
➡️Discoid rash
➡️Serositis
➡️Neurologic disorder
Consider aPL and LAC status in patients w/these manifestations and whether anticoagulation/antiplatelets may be needed

#SLE pts have equal rates of post op CV events compared to TypeII DM pts and ⬆️ compared to matched controls. Esp driven by comorbidities. Should we add ASA post op in SLE pts with CV risks? #ACR21 #abst0128 @RheumNow https://t.co/COdbDxFgBQ

Lupus nephritis mortality from 1999-2019
⭐️-27% age-standardized mortality rate in LN death, though increased trend in last 5 yrs!
▶️ Afr-American 38% of deaths, though 12% of population (RR ~4.5), ⬆️mortality with urbanization

Yikes! Higher rates of lupus nephritis deaths in last 5 years
👉8900 lupus nephritis deaths in USA studied
race/ethnicity and urbanization assc w/ LN mortality
👉highest in blacks and large central metro regions

#ACR21 #Abstr0454 Mortality rate in #lupus nephritis ⬇️by 26% from 1999-2019👏. BUT, racial and residence disparities were concerning; worse in Black, Hispanic, Asian, AI/AN vs White and in large central and medium metro. Residential matters! @RheumNow https://t.co/33vN39acuL https://t.co/XMwN2hXJn7

Race/ethnicity and residence were independent risk factors for LN mortality which was higher in:
➡️Black patients
➡️Those living in central metro areas

⭐️SLE pts ⬆️♀️, older, caucasian, ⬇️ edu than controls
⭐️CI in longer disease, APS

How about lipoprotein a as a potential biomarker for poor renal outcomes in #lupus?
Results of this study by Dr @CaoilfhionnMD and her group show that ⬆Lp(a) is strongly associated w/ proteinuria, ⬇ eGFR and ⬆PGA.
Interesting🤓

Don't wait!
DR. Petri wants us to change paradigm of how we treat aggressive lupus nephritis: early, aggressive, and multimodal. Does not matter if you add on VCS or BLMB, just don't treat LN with MMF monotherapy.
