The Weight of Evidence

Every rheumatologist knows this patient. Her ASDAS is 3.4. Her CRP is normal, her MRI is quiet, and she is on her third biologic. The reflex is to reach for a fourth. Abstracts from ACR Convergence 2026 make a quietly radical case against this reflex. The next advance in spondyloarthritis may come less from a new molecule than from looking properly at everything around the inflammation. The score is not the patient. Five lenses bring that into focus.

Weight
Obesity has moved from footnote to treatment target. In US health records covering 19.9 million people, four in five with PsA lived with overweight or obesity, and obesity carried almost twice the hazard of developing PsA [2015, 2014]. TOGETHER PsA reports at 52 weeks on Wednesday, alongside pooled trial and real world data on adding incretins to biologics [2942, 2298, 0346, 0373]. The question is no longer whether weight matters, but whether we treat it.

Pain
A third of axSpA patients in one cohort show nociplastic features, and a short fibromyalgia screen may stop us escalating for pain that is not inflammatory [1337, 1354]. Meanwhile, early pain relief on a JAK inhibitor may predict who will be doing well a year later [0347]. Pain is not one thing, and our composite scores cannot tell the difference.

History
Why did the last drug fail? Antibodies to a first TNF inhibitor appear to predict a better response to the next biologic [2284]. Registries from the UK, the Czech Republic and the US suggest that function, BMI and distress predict outcome more than mechanism does, and that cycling within one class remains common [1324, 0077, 2288]. Our own 22 year Reading cohort adds to that picture [0352].

The gut
From a bacterial peptide that may keep a pathogenic T cell pool replenished, to a dual antibody designed to remove it, to a simple stool test and a joint clinic with pharmacy at the table, the gut joint axis runs from bench to bedside this year [1027, 2941, 1336, 2287].

Definitions
Who counts as axSpA? The 2025 ASAS SPARTAN criteria meet their first real world tests, and early data suggest they classify noticeably fewer patients than the 2009 criteria [2347]. Classification decides who enters trials, so this matters to all of us.

Where it converges
The threads meet when ASAS presents points to consider for difficult to manage axSpA [2944]. Reassess the diagnosis, confirm inflammation objectively, look for mimics, check adherence, and only then change treatment. In effect, it formalises the lesson of the whole meeting.

Here is what I’ll be doing at ACR26 when a new result crosses my feed this week: I will ask one question - does this help me see the person behind the score. If it does, it is probably an abstract worth remembering.

I will be posting daily from @synovialjoints - please follow along.
