CAR-T Reaches the Synovium: COMPARE Phase 1 in ACPA-Positive RA
Treatment-refractory RA is more common than we sometimes admit. Roughly 11% of patients fail multiple b/tsDMARD classes, and for the ACPA-positive subset, rituximab has long been the fallback. It works, but it rarely produces durable disease-free intervals. CD19 CAR T-cell therapy was always going to be the question that followed the SLE data. Until EULAR 2026, that question had no prospective answer. The plenary session changed that.

Current therapies for rheumatoid arthritis rely on sustained immunosuppression rather than restoration of immune tolerance, with off-drug remission remaining rare.

Chimeric antigen receptor (CAR) T‑cell therapy, long established in hematologic malignancies, is now emerging as a transformative approach in systemic lupus erythematosus (SLE), a disease historically managed with chronic immunosuppression.

#EULAR2026 OP0170 Progress of CAR-T in myositis? Phase 1 of rese-cel autologous CD19 CAR-T (advantage: humanised, 4-1BB Co-stimulatory Domain to enhance cells expansion) showed no safety signals and early efficacy in 4 IIM subtypes. Both anti-synthetase pts relapsed @RheumNow https://t.co/QisldedCti
#EULAR2026 OP Are we closer to controlled trials of CAR-T? Miv-cel KYV-101 autologous CD19 CAR-T reported efficacy and safety profile in 6 patients with severe refractory #RA (median prior bDMARDs: 6) in Phase I COMPARE. Will proceed to Phase II with RTX as a comparator @RheumNow https://t.co/Rb4BFgBDZE

#EULAR2026 OP077 Another promising CAR-T in autoimmunity. In a Phase II study, Rap-Cel autologous CD19 CAR-T (shorter infusion time ~2d & low dose) vs traditional CAR-T showed drug-free improvement in N=6 #IIM & N=6 #SSc patients. Safety similar to other CAR-Ts @RheumNow https://t.co/byNn5X8JGh

#EULAR2026 OP076 Of 50 patients treated with autologous CAR-T, 12% relapsed; median 13 mths. More frequent in #IIM vs #SLE or #SSc. These relapses were significant. Switching to BCMA-CAR or BCMA-TCE restored response. Should BCMA instead of CD19 be targeted initially?@RheumNow https://t.co/lgdsJQm3xY
