Many young rheumatologists do not take vacation in the first year or two as they build their practice, gain knowledge and business acumen and acquire partners or those you can rely on. But to wait many years because of loan debt would be a major mistake. Loans eventually get paid off and should not be at the expense of much needed vacation for you and your family.
