Having to interface with 5 patients in the last few days with ILD and rheumatic disease, 3 with RA, I can only think of the title Clear and Present Danger, though not always initially apparent. (Not so much Jim Morrison's, "The future's uncertain and the end is always near.")
I must say (and before I read your column) when I first heard about Glenn Frey's RA and "acute" (new or re-activated) UC, I thought he required high dose steroids and an anti-TNF agent - hence the pneumonia and his demise. A mix of elements you identified above.
