Dr. Jack Cush reviews the news and journal articles from the past week on

  1. A retrospective study from a Taiwan health insurance claims data analysis looked at the association between Hashimoto's thyroiditis and lupus.
  2. 32 men w/ B27+ #AS (Rx NSAIDs, BASDAI ~5.3) underwent exercise training (10d, 60min) w/ or w/o Pre-Rx w/ whole-body cryotherapy (WBC). WBC had significant drops in hsCRP, sP-Selectin, sVCAM-1, neopterin & signif decreases in BASDAI, BASFI, pain intensity
  3. Overview of Juvenile idiopathic arthritis (JIA) assoc uveitis (JIA-U) seen in 10–15% of JIA pts, higher risk w/ ANA+, oligo-JIA, f& onset ≤6 yrs age. Rx options: topical steroids, MTX, ADA, INFLX, RTX and TCZ (review of TCZ in JIA-U)
  4. Danish Pop. study of frequency of TMJ and oro-facial Dz in 613 pts w/ #JIA followed 4 yrs. Cumulative incidence of TMJ dz was 30%; 21% had arthritis-induced dentofacial deformity & dentofacial dysfunction (52%). TMJ predicted by younger onset, F, ANA+
  5. Survey of 300 Rheums, GI, Derms - concerns w/ new biosimilars - majority say economics arent favorable to switch to biosimilar - Yet majority "very to somewhat” comfortable w/ biosimilars - US: Slow biosimilar uptake due to disappointing costs
  6. Metanalysis of 25 studies, betw 1985 - 2021, showed US average annual visits for RA=5.25 for US rheums (vs 3.3 ex-US) & 4.8 US non-rheums. SLE=3.24 & 12.3 for US non-Rheums. FM=1.8 for US rheums & 0.4 vs ex-US Rheums. Rheum Visits Decreased 1982 to 2019
  7. EHR study of UK Clinical Practice Research Datalink studied 318 hospitalized gout pts showing there is a 30d increased risk of VTE after gout flare aIRR 2.31 (1.39-3.82), that disappeared 31-90d after.
  8. Tigulixostat is a non-purine xanthine oxidase inhibitor studied in 143 gout pts vs PBO with goal of SUA < 5. TIG was effective at 50mg (47%), 100 mg (45% ), 200 mg (62%) vs PBO 2.6% at week 12; w/ more gout flares in PBO Rx pts (13% vs 9%)
  9. Differential Dx of Granulomatous Myopathy: Sarcoidosis, inclusion-body myositis, myasthenia gravis, Crohns, GPA, TB, brucellosis, syphilis, PBC, RA, systemic sclerosis , lymphoma, checkpoint inhibitors, GVHD, natural killer/T-cell lymphoma
  10. S.Korean population based study shows the frequency of pregnancy induced de Quervain's tenosynovitis (DQT) between 2013-17 was 2.1% of all preg women. Risk factors: ≥ 30 yrs, multiple gestation, C-Section, HTN in preg, RA (DM not a risk)
  11. Im not sure this was necessary but - a metanalysis of 10 RCTs w/ 847 OA patients shows colchicine use in osteoarthritis had no efficacy as far as pain reduction (vs control; SMD= 0.17) & no improvement in function (SMD, - 0.37)
