Seropositivity Linked to Bronchiectasis in Rheumatoid Arthritis

Rheumatoid arthritis associated bronchiectasis (RA-BR) is uncommon, and is unrelated to interstitial lung disease (ILD), but like ILD, can substantially impact RA outcomes.

A case-control study from the Mass General Brigham Biobank compared RA-BR (57 cases) and RA controls without RA-related lung disease (n-360) by clinical parameters and CT chest imaging results. 

RA-BR was significantly associated with older age of RA onset (OR 1.37 per 10 years), lower BMI at RA diagnosis (OR 0.94 per kg/m2), seropositivity (OR 3.96); including positive rheumatoid factor (OR 4.40), and positive anti-CCP (OR 3.47). Higher-titer RA-related autoantibodies were associated with higher odds of RA-BR.

These results suggest a pathogenic link between airway inflammation and RA-related autoantibodies.
